3rd Year Residency Training

Continue Meeting with your QI Faculty Mentor

You will continue to utilize the faculty mentor you selected as a guide throughout this final phase of the QI resident curriculum. You will be responsible for initiating meetings with your mentor and will be required to meet with them at least three times throughout the year

Participate in Didactic Sessions

Attend the didactic sessions offered throughout the year to reinforce specific QI methods and tools you have learned. Dates and times for these sessions will be announced and posted on the QI calendar. It is required that you attend, or view, 75% of the presentations throughout the year. If you are unable to attend, the power points for the didactics will be viewable online.

Required Reading/Competency Materials

To continue learning QI methods and tools, you will need to fulfill the list of 3rd year QI Reading Requirements. If preferred, you can check these same readings out from the QI Library located in the Center for Advancing Pediatric Excellence, 4th floor, Medical Education Building. For additional QI learning, please visit the Gossary of QI Terms.

Complete Online IHI QI Modules

You are required to complete the IHI QI modules listed below. Once complete, print off your transcript and give it to the QI Coach. To begin the courses, enter the IHI Website and follow the instructions to access the modules.

Your required modules include:

1st Half of Year

PS 101: Fundamentals of Patient Safety

  • Lesson 1: To Err is Human
  • Lesson 2: Responding to Error
  • Lesson 3: Identifying and Reporting Errors
  • Lesson 4: Error

2nd Half of Year

PS 102: Human Factors and Safety

  • Lesson 1: Understanding the Science of Human Factors
  • Lesson 2: Changes Based on Human Factor Design Principles
  • Lesson 3: Using Technology to Mitigate the Impact of Error

Participate as a "Physician Champion" on a QI Project

To gain experience leading a QI project, it is important to be a physician champion, who leads the QI team click here to learn more about being a physician champion.  It will be important to get guidance from your QI faculty mentor who will be an excellent resource in this endeavor. To begin leading a project you should complete the following things:

  • Identify a QI Project. You can: 1) maintain your team membership on your current QI project but redefine your role as physician champion 2) select an existing project to champion or 3) establish and champion your own project (this will require more effort, but could be very rewarding).
  • Understand the project background. If you choose an existing project, you will need to reference the QI project inventory list in the LCH QI Documents. If you are establishing your own project, begin to understand the problem and develop your own background information by:

    • Writing a brief "problem statement" with baseline date (e.g. chart audits) related to your project (What is the extent of the problem and why is a change important?)
    • Writing a "project aim statement"which includes the project goals (What are you trying to accomplish?)
    • Listing your project measures (How will you know a change is an improvement?)
    • Listing the changes you intend to make (What changes will result in an improvement?)
    • Recruit others, with an interest in your QI project idea, to join your team

  • Utilize your QI coach and data coordinator. This will allow you to review your progress, receive guidance and get feedback on planning and implementing changes and how to collect data.
  • Conduct your QI Project. Use the tools you have learned, and apply the PDSA approach to test changes in clinical settings. When selecting your changes, remember to choose one simple enough to complete a PDSA in a short period of time, preferably within one week. You can reference the PDSA template in the LCH QI Documents for assistance.

Present QI Project at QI Quarterly Meeting

At the June QI Quarterly Meeting, see the Calendar for more details. you will present the QI Project you have "championed". This will be done utilizing a five minute, rapid fire power point presentation using the LCH power point template, found in the LCH QI Documents, and a storyboard visual for attendees to view. You will receive further instructions from the QI program coordinator closer to the June meeting.

Submit a Final Report

Once you have completed your 3rd year QI project requirements, you will submit a final report, click here to choose a template for report.  This will allow you to reflect on your project experience and lessons learned. The completed report should be emailed to your QI faculty mentor and QI coach by the end of May. You will be able to discuss this report at your QI closeout meeting.

Complete QI Competency/Program Evaluation Survey

Complete this survey after completing all QI project activities. This survey will assess your knowledge of QI methods and tools and will be used to evaluate the effectiveness of the QI resident curriculum/program. This survey can be accessed through the LCH QI Documents and should be brought with you to your closeout meeting

Attend Closeout Meeting with QI Coach and QI Faculty Mentor

After completing all of the above requirements, you will need to contact your QI coach and QI faculty mentor by the first week of May to arrange a closeout meeting. During this meeting, you will review your work/portfolio throughout this QI process and the results of your pre- mid- and post-assessment surveys. You should reflect on your experience so you are prepared to give feedback.

Sustainability of QI Project

Upon graduating, it will be important to hand off your QI project to another team or individual who can manage your current progress and sustain your efforts. It is possible a 2nd year resident may be interested in taking on this role. Your QI Coach will help facilitate this process during your closeout meeting.
