Choose a QI Faculty Mentor
Select a QI faculty mentor who will, along with your QI coach, guide you as you begin to put your QI knowledge into practice. These faculty members have all received QI training and understand the methodology. You will need to select a faculty member within the 1st month of your 2nd year and let the QI coach know of your selection so they can inform that individual. You will be responsible for initiating meetings with your faculty mentor and are required to meet with them at least three times throughout the year. A list of QI faculty mentors is posted in the LCH QI Documents.
Complete a QI Competency Survey
Complete the competency survey given to you at the beginning of your 2nd year of residency. This survey, distributed at your orientation will assess your knowledge of QI methods and tools based on your exposure to, and practice of, the information throughout your 1st year of residency.
Participate in Didactic Sessions
Attend the didactic sessions offered throughout the year to reinforce specific QI methods and tools you have learned. Dates and times for these sessions will be announced and listed on the calendar. It is required that you attend, or view, 75% of the presentations throughout the year. If you are unable to attend, the power points for the didactics will be viewable online.

To continue learning QI methods and tools, you will need to fulfill the list of 2nd year QI Reading Requirements. You can also check these same readings out from the QI Library located in the Center for Advancing Pediatric Excellence, 4th floor, Medical Education Building. Complete Online IHI Modules
You are required to complete the IHI QI modules listed below. Once complete, print off your transcript and give it to the QI Coach. To begin the courses, enter the IHI Website and follow the instructions to access the modules.

1st Half of Year:
QI 103: Measuring for Improvement
- Lesson 1: Measurement Fundamentals
- Lesson 2: Displaying Data
- Lesson 3: Learning from Measures
2nd Half of Year:
PS 103: Teamwork and Communication
- Lesson 1: Why are Teamwork and Communication Important?
- Lesson 2: Basic Tools and Techniques
- Lesson 3: Communication During Times of Transistion
- Lesson 4: Developing and Executing Effective Plans
Participates as Team Member on a QI Project
Participate in a current QI collaborative or initiative within the inpatient or outpatient setting of Levine Children's Hospital. This will provide the opportunity to learn more about QI and will help you develop skills needed to participate on, and lead, a QI team. To help you choose a QI project, we have provided a current QI project inventory list, with project description form attached, which can be accessed through the LCH QI Documents. Pay close attention to the project "phase" on the project description form to ensure you choose a project that is in phase 1 or 2. As an alternative, you can initiate a project based on an identified QI need. If you choose this avenue, you will need to contact the CAPE team who will approve your project idea and guide you through that process. Your project selection will need to be made by the first week of August.
When joining a QI project team, it is important to understand the project background. This can be achieved by contacting the project team leader to find out: (1) the "charter" or "aim statement" (2) project measures and (3) the project changes (sometimes called "change concepts" or "change package"). In addition, you will need to attend the QI project team meetings to understand, and participate in, the specific changes being implemented. You are expected to attend as many team meetings as possible and play an active role on your team. Some roles may include: data collection, PDSA developing and implementation or assisting with clerical tasks. These roles will ebb and flow based on the phase of your QI project.
Submit a Final Report
Once you have completed your 2nd year QI project requirements, you will submit a final report click here to see a template for the final report. This will allow you to reflect on your project experience and prepare for the upcoming year. The completed report should be emailed to your QI faculty advisor and QI coach by the end of May. Once you have submitted the report, your QI Coach will contact you to schedule a meeting to review the report. You are encouraged to invite your QI faculty mentor to attend this meeting as well. This meeting will be a good time to also receive guidance in planning for your more extensive, 3rd year QI project
Sustainability of QI Project
If you choose to participate in a different QI project during your 3rd year of residency, you will need to ensure sustainability of your current QI project with your existing team. You can discuss this with the QI Coach who will help facilitate this process.
